If you are new to Monarch Health (even if you saw Dr. Strizich at his prior practice), please enter your basic information through our secure portal.

Transfer your health records

Drop by our office to sign a release form, then we'll request your records be transferred from your previous primary care doctor and/or specialist. This helps make the most of your first visit with Dr. Strizich.

Once we get the ball rolling to transfer your records to Monarch Health, feel free to book an appointment for at least 2 weeks out. If you need to be seen more urgently, just give us a call to discuss: 208-269-5578

If yours isn't listed, please give us a call and let us know, so we can start the process.
We also offer cash-pay discounts and will do the paperwork to bill your insurance even if they are out of network.
